In March 2025, a surgeon in New York successfully removed part of a kidney from a patient in São Paulo. The 1,500-mile procedure, using a modified robotic system over a private network, was a technical triumph. The patient recovered well. But this achievement, while notable, underscores a persistent divide between a laboratory demonstration and a practical medical tool.

Remote surgery is not new. The first major transatlantic operation occurred in 2001. The central challenge then, as now, is latency - the delay between a surgeon's movement and the robot's response. The recent procedure cut this delay to about 80-100 milliseconds, a significant improvement from 24 years ago. However, it required a custom, non-standard network setup. This remains the core obstacle: we lack a universally available network that can guarantee the consistent, ultra-low latency surgery demands.

Beyond engineering, regulatory and logistical walls stand tall. Medical licensing, liability across borders, and the sheer cost of robotic systems in underserved areas are unresolved. While the vision of a specialist operating across continents is powerful, it assumes a fully-equipped operating room and flawless connectivity on the other end. These are scarce in the very regions that need such technology most.

The practical near-term path may be less dramatic. Telementoring, where experts guide local surgeons in real-time, is already improving care without needing to solve the latency puzzle. For direct remote control to become routine, the focus must shift from the robot to the infrastructure. The capability exists. The reliable, widespread network to support it does not. Until that gap closes, remote robotic surgery remains a promise, not a practice.
